The UEMD Resource Hub is built on a coordinated, community-driven model where food is the entry point, and wraparound services create pathways to stability. This Partner Quick Guide outlines how we work together to serve individuals and families through a shared intake system, clear communication, and aligned services. From submitting referrals through our intake link, to coordinated access led by our Navigator, to partner engagement and follow-through, every step is designed to ensure that no one falls through the cracks.

This guide also provides essential information for Hub operations, including access details, scheduling procedures, weekly programming, and partner expectations—so we can function as one connected system, not separate services.

    Provide an overview of your organization's primary programs and services: We offer intensive care management to pregnant moms attending our St. Agnes OB clinics. Patients are able to get lyft rides to and from all OB appointments, pharmacy visits, high risk OB visits, and other clinic visit related to pregnancy or postpartum up to 6 weeks after delivery. We also provide pregnant and new moms with baby supplies and linkage to baby supplies. As well clinical support in managing high risk pregnancies with a diagnosis of anemia, gestational diabetes, and hypertension.

    The Food Project is the flagship program of UEmpower of Maryland, built to confront two urgent challenges in Southwest Baltimore: food insecurity and youth unemployment.

    The Food Project serves as both a safety net and a launchpad for opportunity.

    U Empower of Maryland (UEMD) is a community-based nonprofit resource hub serving Southwest Baltimore. At the UEMD HUB, we identify needs and create solutions by connecting neighbors to wraparound support under one roof—from fresh food access and weekly meal support to benefits navigation, health insurance, housing support, therapy connections, mentorship, workforce readiness, and support for returning citizens.

ALL referrals must go through the Intake Form for coordinated access:

 

PARTNER EXPECTATIONS

  • Use Intake Link for ALL referrals

  • Respond to referrals within 48 hours

  • Communicate outcomes back to Navigator

  • Maintain professional + trauma-informed environment

  • Support a shared system — not siloed services

Step 1: Intake

  • Any partner can submit a client through the intake link

  • This is the entry point to the system

Step 2: Navigator (Sheena)

  • Receives all intakes

  • Conducts coordinated access

  • Identifies full scope of needs

Step 3: Partner Connection

  • Referrals sent to specific partners

  • Partners contact clients directly

Step 4: Real-Time Support

  • Clients receive info, follow-up, and next steps

Step 5: Tracking & Accountability

  • Everything is tracked

  • Follow-ups happen

  • Outcomes matter

  • $50/hour → Half Hub

  • $100/hour → Full Hub

SECURITY

All partners operating within the UEMD Resource Hub are responsible for ensuring their own security needs are met. While the building is secured, U Empower of Maryland does not provide on-site security personnel.

Partners who feel additional security is necessary for their programming or operations are responsible for arranging and covering the cost of their own security services. Contact Ms Neka for scheduling security (443) 800-2624.

For any event, program, or gathering with more than 25 attendees, partners are required to hire appropriate security personnel to ensure the safety of all participants.

U Empower of Maryland is not liable for incidents related to partner-hosted activities where additional security measures are deemed necessary.
